Patient Information

Dr. Marshall and staff are dedicated to providing you with a pleasant visit and results that you’re proud to show off. Our administrative staff is ready to help you with questions about scheduling, financial policy and insurance, to make that part of the process as simple as possible.


First Visit

Your initial appointment will consist of going over your medical history, taking any necessary x-rays, a consultation explaining your diagnosis and your treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays if applicable.

  • A list of medications you are presently taking.

  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to any dental work (i.e. diabetes, high blood pressure, artificial heart valves and joints, heart murmur, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e. antibiotics for pre-med.)

X-Rays

If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

To help you save time, we have provided our fillable patient forms online so you can fill them out before your visit! If you have questions, then feel free to call our office during regular business hours to speak with one of our staff.


Financial Policy

Patient Billing

For your convenience we accept Visa, MasterCard, American Express and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at 408-262-6916. Many times, a simple telephone call will clear any misunderstandings.

We also offer financing through CareCredit.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement if you have any balance due in our office. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan but this must be done prior to the actual procedure.


Insurance Information

We make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. We are happy to call and confirm if you can or cannot use your insurance coverage in our office. Please be aware that your insurance plan is an agreement with you and your employer and not us. We are not contracted with any insurance and cannot write off any patient portions. If we have any problems with your insurance processing your dental claim, we will reach out to you to help us with any documentation they may need. If there is a co-pay, it will be due at time of service and if there is a balance on the account we will send you a statement home for payment. Again any remaining balance after your insurance has paid is your responsibility.
